History or strong clinical suspicion of ethylene glycol poisoning and at least 3 of the following criteria:- Arterial pH less than 7.3- Serum bicarbonate level < 20 mEq/L (20 mmol/L)- Osmolar gap > 10 mOsm/kg H2O- Urinary oxalate crystals present
